SEN Assessment Form Part 2

Please note this is from the pre-2014 SEN system in England.

Further to the earlier blog ‘The SEN Assessment Form – Yikes!’ I thought it might be useful for some people to provide a template to help people get their thoughts down so you’re not facing a blank piece of paper. You can download it below, in MS Word format. If you need it in pdf format, just email info@specialneedsjungle.co.uk and I’ll be happy to send you one.

**Although the system in England has now been superseded by Education Health and Care Plans, the preparation you need to make will be the same so you can still use this form. Additionally, it will be the same for those in Wales and Scotland, and can be used to help you get started for applying for SEN help in other parts of the world as well!**
